What Makes DIEP Breast Reconstruction Unique?
DIEP reconstruction revolves around an advanced microsurgical technique. Unlike other forms of breast reconstruction that may utilize breast implants, DIEP taps into the body’s own resources. The procedure involves transferring skin, fat, and necessary blood vessels from the patient’s abdomen to the chest area. This autologous transfer (meaning it comes from the patient’s body) ensures a natural feel and appearance to the reconstructed breast. The best way of thinking of this procedure is as a transplant within the patient. This surgery avoids the complications and concerns that are associated with implant-based reconstruction. DIEP reconstruction also yields an abdominal contouring similar to a tummy tuck.
What truly sets DIEP apart is its meticulous preservation of the abdominal muscles. This differs from other methods like the TRAM flap, where all or a portion of the abdominal muscle might be taken during the surgery. By preserving these muscles, DIEP offers patients an advantage in terms of strength retention, post-surgical recovery, and reduced likelihood of abdominal hernias or bulges.

I adore the staff and surgeons at this office. I had thought about having plastic surgery on and off throughout my life. I never actually did it, something else always took priority. Until I was diagnosed with stage three breast cancer that had metastasized. I was faced with the fact that I needed a mastectomy. I was one thousand percent against DIEP flap surgery which my oncologist kept telling me was my best choice. At this point in time I had so much information being thrown at me and I felt completely out of control of anything in my life. I called and made an appointment with Dr. Chris McClendon because I wanted implants. I was called back and was told that Dr. Chris had reviewed my chart and thought I would be better with Dr. Taylor McClendon.
I went in to see Dr. Taylor McClendon he was so nice and genuine. One of the first things he said after we introduced ourselves and spoke about my situation was “So you’re thinking about DIEP Flap reconstruction!”, turns out that was on my referral from my oncologist. I told him very frankly that I had absolutely NO desire to have the DIEP Flap reconstruction. He very gently asked me to be opened minded and let him explain this option as well as implants and then I could decide which one I would like to have. He explained both, answering every question I or my husband had. His passion for what he does and the breast cancer patients in our community was so clearly betrayed in the way he expressed himself and spoke to us. We left our appointment, I looked over at my husband and said “I will be getting the DIEP Flap Reconstruction surgery and he will be my surgeon.”
I had my DIEP Flap reconstruction in December of 2022 and it was absolutely the BEST decision I could have made for myself and my body. I would and have recommended this procedure to any breast cancer patients who unfortunately have to have a mastectomy. I also would and have recommended Dr. Taylor McClendon to patients in our community and as far away as California. He is absolutely amazing and is a perfectionist. He will be more concerned about little things then you yourself. Let me put it this way, I am thinking about moving out of state. I have already told my husband I am willing to find all new doctor’s except this one! I will travel hours and through states to see him. This is a HUGE decision at one of the worst moments in your life. When you find a doctor/surgeon that truly listens to what YOU want and you trust him completely with your life (this is not a minor surgery); you are willing to share with the whole breast cancer community how amazing he is at what he does! I know this isn’t the only procedure he does but it is the only one I have and can speak on from experience. Telling the world that you have had a mastectomy isn’t really the most comfortable thing to share, however, in this situation I am willing to in hopes of helping another patient going through the same situation.

What can I expect with my recovery?
The recovery timeline is very similar to a tummy tuck. The main difference is that we do not perform this surgery on an outpatient basis. We admit our patients to the hospital for a period of close observation that usually lasts ~ 48 hours. The vast majority of our patients are ready for discharge on the morning of post-operative day 3, thanks to the use of an Enhanced recovery protocol. Patients can expect to return to normal, low-intensity activities by the two-week mark. We then ask that patients only resume heavy lifting or abdominal exercises at week 6 (this is when the scar tissue is strongest).
How many surgeries can I expect?
The answer depends on each patient’s individual diagnosis and reconstructive timeline. Treatments such as radiation and chemotherapy may prevent a patient from having their DIEP reconstruction at the time of the mastectomy.
While performing the DIEP flap, our goal is to achieve a high level of symmetric and aesthetic outcome in order to reduce the likelihood of any revision surgery. Should a revision be necessary, it is most often an outpatient “touch-up” surgery where minor asymmetries are corrected. These procedures often require minimal downtime.
